Although infants in the neonatal intensive care unit are often at a greater risk of injury caused by a wide array of errors, medication errors in the NICU are particularly common. Neonates (especially premature or low-birth-weight babies) are particularly vulnerable to the adverse effects of medication errors due to their inability to communicate and their limited ability to "buffer" such mistakes.
Common causes of medication errors in NICU:
Because medication must be administered according to the infant's weight, any and all doses must be specifically calculated (and may need to be updated as the infant gains or loses weight). Due to the low weight of infants in NICU, there is a very real risk of "tenfold medication errors" where a misplaced (or overlooked) decimal point could result in a dose ten times higher than recommended. Almost half (47%) of all errors in the NICU are directly attributed to the wrong medication, dosage, schedule, or infusion rate.
Medication errors in the NICU also commonly occur in the route of administrating the drug (e.g., breast milk mistakenly being given intravenously). One reason for this is that neonatal feeding tubes and intravenous lines (and their hubs) are often similar in appearance and size. Patient misidentification is another serious concern when it comes to infants, as one study found that around 25% of serious NICU medication errors involved misidentification.
